Stephen Allum killed his wife Thelma in October 1997. A few weeks previously he had been admitted to a psychiatric ward following a serious assault on another relative. He was eventually convicted of manslaughter on grounds of diminished responsibility. This inquiry was commissioned to investigate the care and treatment that he received while in contact with the NHS and other agencies. Chairman of the inquiry: Anselm Eldergill.
